Dehydroepiandrosterone sulfate (DHEA-S), also known as prasterone sulfate, is a naturally occurring, endogenous androstane steroid and neurosteroid and the 3β-sulfate ester of dehydroepiandrosterone (DHEA). As the sodium salt, prasterone sodium sulfate, DHEA-S is used as a pharmaceutical drug in Japan in the treatment of insufficient cervical ripening and cervical dilation during childbirth.
Synthesis
Dehydroepiandrosterone sulfate is produced by the addition of a sulfate group, catalyzed by the sulfotransferase enzymes SULT1A1 and SULT1E1, which also produce estrone sulfate from estrone. DHEA sulfate can also be back-converted to DHEA through the action of steroid sulfatase.
In the zona reticularis layer of the adrenal cortex, DHEA-sulfate is generated by SULT2A1.This layer of the adrenal cortex is thought to be the primary source of serum DHEA-sulfate. DHEA sulfate levels decline as a person ages as the reticularis layer diminishes in size.
Use as biomarker
Dehydroepiandrosterone sulfate levels above 1890 micromol/L or 700-800 µg/dL are highly suggestive of adrenal dysfunction because DHEA-S is made exclusively by the adrenal glands. Presence of DHEA-S is therefore used to rule out ovarian or testicular origin of excess androgen.
Therapeutic use
The Endocrine Society recommends against the therapeutic use of DHEA sulfate in both healthy women and those with adrenal insufficiency, as their role is not clear from studies performed so far. The routine use of DHEA-S and other androgens is discouraged in the treatment of women with low androgen levels due to hypopituitarism, adrenal insufficiency, menopause due to ovarian surgery, glucocorticoid use, or other conditions associated with low androgen levels; this is because there are limited data supporting improvement in signs and symptoms with therapy and no long-term studies of risk.
In otherwise elderly women, in whom an age-related fall DHEA sulfate may be associated with menopausal symptoms and reduced libido, DHEA sulfate supplementation cannot currently be said to improve outcomes.

Dehydroepiandrosterone sulfate (DHEAS) is a metabolite of dehydroepiandrosterone that is the major secretory product of adrenal glands and is the predominant circulating precursor for active steroid hormones in humans. For example, in the fetoplacental-maternal unit, DHEAS acts as the primary precursor for placental estrogen biosynthesis. A normal circulating concentration of DHEAS is ~10 µM in young adults and is dramatically increased in some adrenocortical disorders.

STABILITY AND STORAGE RECOMMENDATIONS:
When dehydroepiandrosterone sulfate, sodium salt, [1,2,6,7-3H(N)]- is stored at -20oC in its original solvent and at its original concentration, the rate of decomposition is initially 2% for 4 months from date of purification. Stability is nonlinear and not correlated to isotope half-life. Lot to lot variation may occur.

What It Is
Dehydroepiandrosterone-sulfate (DHEA-S) is a steroid hormone produced mainly by the adrenal cortex, the outer part of the two adrenal glands that are located just above the kidneys. Though its an androgen (a male-type sex hormone), its produced by both males and females.
DHEA-S serves as a building block for making the male sex hormone testosterone and the female sex hormone estrogen. A DHEA-S test measures the amount of the hormone in the bloodstream.
DHEA-S levels are high in newborn babies, then quickly drop. They rise again during puberty, when they play an important role in triggering the development of pubic and underarm hair.
Why Its Done
Doctors may order a DHEA-S test when they want to check the functioning of the adrenal glands, as in the case of a child who shows signs of DHEA-S overproduction. For both boys and girls, this may mean the appearance of puberty-related characteristics earlier than expected (precocious puberty), particularly the early appearance of pubic or underarm hair.
Significant overproduction or DHEA-S in a girl may result in the development of masculine characteristics such as facial hair, deeper voice, acne, increased muscle mass, and irregular or absence of menstrual periods. An excess of DHEA-S may be caused by congenital adrenal hyperplasia (a condition present from birth in which the adrenal glands make too much androgen and not enough of other steroid hormones) or some other adrenal problems. Low levels of DHEA-S may be present if the adrenal glands are damaged or diseased.
The test may also be used, with other hormone tests, to rule out certain diseases of the testes or ovaries because DHEA-S is also produced (normally in small amounts) by these organs. The test can also help diagnose damage or disease of the pituitary gland because adrenal production of DHEA-S is ultimately controlled by pituitary hormones.
Preparation
No special preparations are needed for this test. In adolescent girls, doctors may request that the test be performed either before or after a menstrual period, as menstruation may affect hormone levels.
